IRS type right of redemption...

In other words, if I were to purchase a property on the courthouse steps that was foreclosed on by the 1st mortgagor, and there was a second mortgage and a federal medicare lien behind it (3 liens total), would the medicare lien still have a 120 day right of redemption similar to an IRS lien?

No, there are no other governmental entities that I know of that have a right of redemption like IRS has.
